Antibiotics may not work in most cases of diarrhea as diarrhea mostly results due to viral infections. Even severe diarrhea may also be cure in few days without using antibiotics. Some times use of antibiotics makes the situation worse especially in the case, when diarrhea is caused by E coli bacteria.

Bismuth subsalicylate (Pepto-Bismol) contains two active ingredients- bismuth and salicylate (aspirin). Bismuth compounds are effective only in traveler’s diarrhea and the treatment of H.pylori. It is not clear how bismuth subsalicylate would work with the only exception of traveler diarrhea. It is like an antibiotic that affects bacteria causing diarrhea. Bismuth may lower the water secretion by the intestine and anti-inflammatory salicylate reduces the water secretion.
